Enrollment Requirement for J1 or F1 Visa Holders

The University requires you to be enrolled in the Premier Plan upon registering in specific credit hour and degree course work. The charge for the Plan is a required fee on your student account. There are very few exceptions to waive the Plan, e.g., if you are covered under a group employer or group embassy plan that meets the University’s Requirements.
To request an exception, complete the applicable Student Health Insurance Acceptance or Waiver Form and, per the last question, upload a copy of your Certificate of Coverage issued and insured by a US insurance company and written in English. The documentation must clearly demonstrate your other policy meets the University’s requirements; is not an excluded plan; and, that your group employer or group embassy plan includes the provisions below for Student Health Insurance (SHI) to grant you a waiver of the Premier Plan.
- Covers medical evacuation and repatriation to your home country at 100%
- Has a deductible not to exceed $500 per illness or injury
